   Send check, money order, or credit card information with completed registration form to:

   Call 718.951.4141, Monday through Friday. Be ready to provide course number and credit card information.
   
   
4. By fax
   Fax 718.951.4285, 24 hours a day, 7 days a week. Send a completed registration form, including credit card information.
