Room Change Request
To e-mail the Room Change Request Form, make sure that you have the most recent version of Adobe Reader on your computer. You may download it free.
- Right-click on this Room Change Request Form link and choose "Save target as..." to save the fillable form on your desktop.
- Find the saved form, click on it to open it, and edit it.
- After you complete the form, print form to PDF and rename using SUBJECT_COURSE_SECTION and save it to the same location used in Step 1.
- E-mail the completed form to your chairperson as an attachment.
- Chairperson should review the request and if approved, forward the form to the Scheduling Office.
