Room Change Request

To e-mail the Room Change Request Form, make sure that you have the most recent version of Adobe Reader on your computer. You may download it free.

  1. Right-click on this Room Change Request Form link and choose "Save target as..." to save the fillable form on your desktop.
  2. Find the saved form, click on it to open it, and edit it.
  3. After you complete the form, print form to PDF and rename using SUBJECT_COURSE_SECTION and save it to the same location used in Step 1.
  4. E-mail the completed form to your chairperson as an attachment.
  5. Chairperson should review the request and if approved, forward the form to the Scheduling Office.
